All student athletes are expected to complete a number of forms/waivers prior to each season of play.

Students will be requried to complete:

   1. Fillable Online Medical and Code of Conduct Form

   2. Printable Extracurricular Activity Release of Information Form (Mandatory by district, no exceptions)

   3. Printable Higher Risk Consent and Release Form

   4. Printable Volunteer Driver Form

   5. Printable Player Information Sheet
