Extra Curricular Consent and Participation Form (MANDATORY)
Effective September 2023, all students who participate on extra curricular sports teams must complete this mandatory form from the district.
The form confirms that you acknowledge that some personal student information will be shared with BCSS for the purposes of registering athletes. Your completion and consent helps ensure we are all compliant with the BC Government's Freedom of Information Policy (FOIPPA).
Until this form is complete, students will not be allowed to participate in school sports activities.
