For those athletes playing rugby and/or hockey, the high risk consent and release form must also be completed.

#4 - Volunteer Driver Form

All students should complete the volunteer driver form in case they or their parents drive to any sporting events.

#5 - Player Info Sheet

While not mandatory, coaches would appreciate if you complete the player info sheet.
